Sriwijaya Air Flight 182 was a scheduled domestic passenger flight from Jakarta to Pontianak, Indonesia. Five minutes after departing from Soekarno–Hatta International Airport on 9 January 2021, the Boeing 737-500 experienced an upset and crashed into the Java Sea off the Thousand Islands, killing all 62 people on board. A search of the area recovered wreckage, human remains, and items of clothing. The flight data recorder (FDR) was recovered on 12 January, and the data storage module of the cockpit voice recorder was recovered on 30 March.[5]
During the search, Indonesia's National Transportation Safety Committee (NTSC) used the available data from Flightradar24, and hypothesised that the plane's engines were still operating upon impact. It was known that the autothrottle on this aircraft had malfunctioned a few days earlier, and one line of investigation was whether this might have contributed to the accident.[6]
A preliminary report released on 10 February 2021 suggested problems with the plane's autothrottle; the thrust lever for the left engine reduced thrust as the aircraft climbed, while the thrust lever for the right engine remained fixed.[7] On 10 November 2022, the NTSC published the final report of the investigation, concluding that the crash had been caused by a combination of a faulty autothrottle and pilot error.[8][9]

Postflight investigation revealed that City of Edinburgh's problems had been caused by flying through a cloud of Volcanic Ash
from the eruption of Mount Galunggung. Because the ash cloud was dry, it did not appear on the weather radar, which was designed to detect the moisture in clouds. The dust in the cloud deprived the engines of sufficient oxygen from the atmosphere to maintain combustion, sandblasted the windscreen and landing-light covers, and clogged the engines. As the ash entered the engines, it melted in the combustion chambers and adhered to the inside of the power plant. As the engine cooled from inactivity, and as the aircraft descended out of the ash cloud, the molten ash solidified and enough broke off for air to again flow smoothly through the engine, allowing a successful restart. The engines had enough electrical power to crank, ignite, and restart because one generator and the on-board batteries were still operating.
*On 24 June 1982, the route was flown by the City of Edinburgh, a Boeing 747-236B registered as G-BDXH. The aircraft flew into a cloud of volcanic ash thrown up by the eruption of Mount Galunggung around 110 miles (180 km) south-east of Ja-k-ar-ta, Indonesia, resulting in the failure of all four engines. Partly because the event occurred at night, obscuring the cloud, the reason for the failure was not immediately apparent to the crew or air traffic control. The aircraft was diverted to Jakarta in the hope that enough engines could be restarted to allow it to land there. The aircraft glided out of the ash cloud, and all engines were restarted (although one failed again soon after), allowing the aircraft to land safely at the H-al-im Per-da-na-k-us-u-ma Airport in J-a-k-ar-ta.
The crew members of the accident segment had boarded the aircraft in K-u-a-la Lum-p-u-r, while many of the passengers had been aboard since the flight began in London.[2]

12 July 2013; Ethiopian Airlines 787-8; ET-AOP; flight 1354; London Heathrow Airport: While the aircraft was parked, unoccupied, and unpowered, and overheating battery associated with the emergency locator transmitter (ELT) caused a fire in the upper part of the rear fuselage. The fire severely damaged the fuselage, but did not cause any injuries or deaths. The AAIB investigation did not determine whether the overheating was caused by a release of energy within the lithium-manganese dioxide batteries, or if was due to an external mechanism like an electrical short. This particular ELT was entirely independent of the aircraft's electrical power system. This event was significant because this model of ELT is used on a wide range of aircraft, and of the roughly 6,000 units produced by the manufacturer, this event was the first recorded instant of a significant thermal event.

Recent US crashes - 9 November 2018; Fly Jamaica 757-200; N524AT; flight 924; Georgetown, Guyana: The aircraft was on a scheduled international flight from Georgetown, Guyana to Toronto, Canada. The crew experienced a hydraulic system failure during climb, stopped the climb at FL200, and elected to return to the departure airport. The aircraft had an excursion off the right side of the runway, coming to rest just short of the end of the runway off to the right. The right main landing gear collapsed, the right engine separated from the aircraft, and the right wing was damaged. All eight crew members survived, and one of the 120 passengers was killed.
